Obverse: Half-figure of the king with the commander's sash; in armor; facing right; holding a sword and royal apple; the Order of the Golden Fleece on his chest. Lettering: SIGIS III D G REX POLO M D LIT RVS PRVS MAS; finished with a decorative cross. Reverse: Crowned; nine-field coat of arms; surrounded by a chain with the Order of the Golden Fleece; on the sides the initials II (Jacob's Wardajna) and the date 16-28; below the coat of arms of Polkozice in the shield.
