Obverse: half-figure of Sigismund III Vasa facing right. epaulet with two rows of small balls with crosses between them. The sash contains only the spheres. Two-bar crown with cap, with one row of jewels . Apple without decoration with two bands of jewels. The king holds a narrow sword with a tapering head with two shavings. The sash is linear. The sash is made up of three folds of matter arranged in parallel. The bow with one ribbon without a knot. The chain of the Order of the Golden Fleece contains chairs made up of two bails, the longer ends of which are intertwined and wrapped inward. The flint, which together with the flint forms the link of the chain, contains a large oval gemstone from which two bundles of rays emerge, in the outer ring the legend:

Reverse: A five-field shield with the coats of arms of the Crown, the Grand Duchy of Lithuania and the coat of arms of the Vasa family, crowned with a crown with a closed rim with 4 cross-shaped jewels, without fleurons, with a reticulated background, topped with a royal apple; On the sides of the shield the date 16-22 and labrys in the shape of snails, in a corded rim, in the outer rim a legend punctuated by the coat of arms of Sas Treasurer Nicholas Danilovich in an ornamental shield:
