[VIRTUTI Militari]. An unfilled form entitled "Application for the decoration with the order 'Virtuti Militari' in accordance with the Resolution of the Provisional Chapter dated 23. I. 20 r.".
Double-sided printing on 4 pages form. 35.5x21.6 cm. The form was printed at the Chief Command's Graphic Works in 1920. Blanket with almost no entries, only short notes in pencil and ink in three fields. The Order of Virtuti Militari, established by King Stanislaw August in 1792, was resurrected by the Legislative Sejm of the Second Republic of Poland on August 1, 1919. The Head of State, Józef Piłsudski, on January 1, 1920, appointed the first Provisional Chapter of the Order, which three weeks later drew up the model of the document presented here. Transverse folding mark, minor stains.
