The badge was awarded to soldiers who had served in the regiment: during operations at the front for six months, and in the reserve battalion for a period of two years. In peacetime, to professional officers and non-commissioned officers after two years of service in the regiment, and to privates and non-commissioned officers after compulsory active service. It was awarded by the regimental commander at the request of the company commander, approved by the battalion commander and deputy regimental commander.

Front: on a laurel wreath a cross, on whose shoulders the inscription 12 LISTOP./GRODZIEŃSKI/1918 R./P. STRZ.; centrally an overlay in the shape of the crowned coat of arms of the Polish-Lithuanian Commonwealth with a centrally placed white enameled shield with a red cross.
